Licensed Practical Nurse (LPN) Lead – Primary Care Clinic – St. Anne Campus – Full Time – Days

This is a Contract position in Raceland, LA posted April 4, 2021.

This job provides individualized, direct nursing care of assigned patients under the direction of a registered nurse, mid-level provider or physician. Assists in data collection and implementation of the delivery of health care services, and assists other members of the healthcare team in providing the highest quality personalized patient care. Maintains a positive work relationship with the health care team, and exhibits the skills to provide care and service with individuals of all assigned age groups.

Education 

Required – Graduate of an accredited School of Practical Nursing.

Work Experience

Required – 2 years LPN experience or one year of LPN experience with one year supervisory experience.

Preferred – Experience in ambulatory care.

 
Certifications

Required – Current State of Louisiana Licensed Practical Nurse License

Basic Life Support (BLS) from the American Heart Association

Knowledge Skills and Abilities (KSAs)

  • Must have computer skills and dexterity required for data entry and retrieval of patient information. Must be proficient with Windows-style applications and keyboard.
  • Effective verbal and written communication skills and the ability to present information clearly and professionally to varying levels of individuals throughout the patient care process
  • Good interpersonal skills

Job Duties

  • Provides safe, effective nursing care to ensure an effective patient visit.
  • Answers patient questions; relays critical information and escalates issues.
  • Completes appropriate medical/legal documentation.
  • Administers medication to patients in a safe manner.
  • Maintains professional and technical knowledge and licensure through continuing education and professional development.
  • Coordinates day-to-day clinical tasks to ensure efficient operation of the department.
